A Mistake is what will be created when Paper Mario attempts to cook something with a non-existent recipe. The Mistake is the Paper Mario games will restore Mario's HP and his FP by a single point.

In Smash, the Mistake is not quite as helpful than in the Paper Mario games. It hides itself amongst the other food items, but upon it's consumption it will cause the player who ate it to choke for a brief second or two and they will also be dealt 3%-5% damage.
